Digging through the Liverpool documentation this is likeley the Stevenage 5 hybrid panel (https://hep.ph.liv.ac.uk/twiki/bin/view/CleanroomTestStands/ABChcc)

All hybrids came with misplaced C7-C12, either none were placed or all had incorrect alignment. Similarly every hybrid had RP1 and RP2 with incorrect orientation.
